722 - Inpatient rehabilitation for the prevention and management of delirium in the acute care settling: meta-analysis and scoping review

Objectives: To evaluate the role of inpatient rehabilitation in mitigating delirium in the acute care setting.

Design: Meta-analysis and scoping review.

863 articles were screened using specific MeSH terms “delirium”, “ICU”, “rehabilitation”, “Physical therapy” and “occupational therapy” and “nonpharmacological treatment” from four electronic databases - PubMed, Embase, Scopus, and Google Scholar and narrowed down our search to 33 articles. Of these, 9 randomised controlled studies that met our inclusion criteria were included in the meta-analysis.

Results: Patients in control group have twice or higher the odds of having delirium (Fisher’s exact test, p < 0.05) in majority of studies. Patients in control group demonstrated to be at a higher risk of delirium with a relative risk of 1.2 or higher in majority of studies (Fisher’s exact test, p < 0.05). Studies that reported the summary (mean/median) duration of delirium (days/hrs) also revealed that the patient in control group had twice or more the duration as that of the intervention group. A scoping literature review revealed that multi component interventions including early cognitive rehabilitation, physical therapy and occupational therapy was associated with significantly improved neurocognitive function in the acute setting.

Conclusions: Inpatient rehabilitation is beneficial for the prevention and management of delirium in the acute inpatient setting
