575 - Patient Reported Outcomes Following Intra-Articular Corticosteroid Injection of Hip: A Retrospective Review

Objectives: Hip osteoarthritis is a prevalent cause of pain and disability. Fluoroscopic-guided intraarticular (IA) corticosteroid injections (CSI) are a commonly performed procedure. Studies have examined the efficacy of these injections and prognostic factors, but few have examined their patient reported outcomes (PRO). This study sought to determine the long-term efficacy of hip IA CSI on PRO and predictive factors for PRO.

Design: Retrospective review of all patients (9/2013 – 3/2020) who underwent hip IA CSI by an interventional spine physician. Descriptive statistics of baseline characteristics were computed. Days of relief and change in PRO (VAS, PROMIS Global Health (GH) Physical (PH) and Mental Health (MH), and PHQ-9) from baseline to post-injection follow ups (months 1, 3, 6, 12, and 24) were estimated using mixed-effects linear regression models. To examine factors associated with change in PRO, mixed-effects linear regression models were fit. Multivariable models were constructed with predictors that were significant at P < 0.05 in unadjusted models. In all models, we included a random effect for patient.

Results: The average pre-injection VAS was 7.30 (N=395) and significantly decreased to 4.60 (N=327), 3.87 (N=118), 3.00 (N=27), and 2.58 (N=6) at subsequent follow-ups. To first follow-up, days of relief was significantly higher at later follow-ups. PROMIS-GH MH scores did not significantly change while PROMIS-GH PH and PHQ-9 scores were significantly improved. In multivariable models, only patients with opioid use and black race were associated with significantly higher VAS scores.

Conclusions: We demonstrated patients receiving hip IA CSI had sustained significant improvements in VAS and days of relief to one year and in PROMIS-GH PH and PHQ-9 scores to 2 years. Our results agreed with previous studies that revealed opioid use and black race can be associated with higher VAS post-injection. Future studies should investigate how to improve those outcomes to decrease disparities in healthcare.
