Case Diagnosis: Traumatic Bilateral Quadriceps Tendon Rupture

Case Description: This is a case report of a 34-year old obese male with HIV well-controlled with undetectable viral load who presented with traumatic bilateral quadriceps tendon rupture following slip on ice. The patient’s surgery was delayed fifteen days and then performed. He completed inpatient rehabilitation and experienced functional recovery.

Discussions: Bilateral quadriceps tendon ruptures are rare, typically occurring in middle aged or older patients with history of chronic disease or steroid use, and commonly present with obesity, which can make the physical exam less conclusive. These injuries tend to be the result of trauma. The physical exam coupled with radiographic imaging lead to the diagnosis of bilateral quadriceps tendon ruptures and visually confirmed on surgical approach.

Conclusions: Traumatic bilateral quadriceps tendon ruptures are rare in young males. They tend to be the result of trauma and in those with chronic disease. Our case is a particularly rare presentation of traumatic bilateral quadriceps tendon rupture in a young male and highlights obesity as a risk factor.
