882 - The Utilization of Osteopathic Manipulative Therapy for the Management of Chronic Pelvic Pain in Women

Objectives: CPP has a prevalence from 5.7% to 26.6% worldwide. Women with CPP have greater incidence of pelvic floor, abdominal, and hip related muscle tenderness. There are several noninvasive therapies for the treatment of pelvic myofascial dysfunctions such as osteopathic manipulative medicine (OMT).

Design: This is a systematic review of which PubMed was searched for the following terms "Osteopathic Manipulative Treatment and Chronic Pelvic Pain " and "Osteopathic Manipulative Therapy and Chronic Pelvic Pain." The inclusion criteria for this study included dysmenorrhea, pudendal neuralgia, and vulvodynia. The exclusion criteria for the study consisted of uterine bleed, pre and postpartum low back pain, IBS, studies that included men, and studies not in English.

Results: Of the 6 studies examined, benefits of OMT were demonstrated on dysmenorrhea, vulvodynia, and pudendal neuralgia induced CPP. OMT treatments were directed toward the pelvic diaphragm, sacrum, sacroiliac joint, and sacrospinous ligament. Within our research we found, one study discussed a successful combined treatment modality of soft tissue, muscle energy, HVLA, and MFR for dysmenorrhea. Another single-blinded randomized controlled trial found visceral manipulation and diet control together to successfully manage dysmenorrhea. Another study showed vulvodynia to be successfully treated using MFR, BLT, and cranial techniques in two studies. Pudendal neuralgia was successfully treated with MFR and BLT in one study. The outcome measures were variable. In all of the studies, there was an improvement in the patient's baseline in pain, Quality of Life Questionnaire, or in EMG activity.

Conclusions: Overall, there is sparse research for the treatment of CPP and even less known about the efficiency of OMT he management of this condition. There are a variety of potential OMT techniques and location targets for the management of CPP. When considering the positive impact OMT has on CPP, it is advantageous that this therapy be employed in the clinic.
