Despite the known benefits of a successful transition program, there is a lack of consensus on the necessary components of an effective and sustainable practice model in transition of healthcare from pediatric to adult medicine. In this discussion, we will present recommendations for a best practice model for transitioning the care of aging patients with early onset disability. We will first provide an overview of the current state, then discuss recent literature on the benefits of such programs. Skills needed to provide uninterrupted, coordinated, developmentally appropriate, psychosocially sound and comprehensive care will be outlined. The challenges will be presented through the perspectives of both the patients/families and the providers. The session will end in a group exercise where participants will work through a challenging case presentation. Through a multifaceted and interactive discussion, participants will learn the skills and strategies necessary to excel in their respective roles as practicing physicians and healthcare leaders, which will then translate into successful career advancement.
