Medical Director, Care Coordination

Dr. Agha works at SSM Health, a 4 state, $9 billion Integrated Delivery Network, as the Medical Director of Care Coordination for St. Joseph's Lake St. Louis. He works with hospital leadership, clinicians, case management, utilization management, and social work to provide care coordination via implementation of new care processes, medical necessity and utilization management reviews, performing peer-to-peer discussions with medical directors at insurance companies, along with clinical documentation improvement. He's also taken on roles to reduce readmissions and standardize multi-disciplinary rounds in the St. Louis region.
Dr. Agha volunteers for numerous roles, including the American Hospital Association, the Missouri Chapter of the American College of Healthcare Executives as the Physician Engagement Committee Chair, Health Information Managements Systems Society Advocacy Co-Chair for the Midwest Gateway Chapter, the American Academy of Physical Medicine & Rehabilitation’s Diversity & Inclusion Committee and the Association of Academic Physiatrists' Leadership Development and Recognition Committee.
