Objectives: The objective of this meta analysis was to determine how effective BMAC is as an adjunct to arthroscopic labral repair in improving patient outcomes.

Design: A systematic review and meta-analysis of multiple databases was completed following PRISMA 2020 guidelines to assess the efficacy of BMAC in acetabular labrum tear repair post-hip arthroscopy. Included were studies with various designs, focusing on functional outcomes after BMAC augmentation measured via IHOT-33. Our initial search resulted in 459 studies, 221 after duplicates were removed, 8 for full text approval, and 3 studies for final inclusion. Exclusions involved unavailable full texts, ongoing studies without data, and studies without BMAC use, studies without isolated hip data, and studies without functional assessment at 12 months.

Results: The pooled analysis of IHOT-33 scores from multiple studies pre and 12 months post intervention reveals a significant and substantial improvement in patient-reported outcomes, as demonstrated by a p-value of < 0.001 and an effect size of Cohen's D = 1.63. This supports BMAC as an effective adjunct to functional outcomes following arthroscopic repair.

Conclusions: While there is much research on BMAC at the knee, this is the first meta-analysis regarding the use of BMAC at the hip, and as an adjunct to acetabular labral repair. The results suggest that BMAC should be considered by surgeons as an adjunct when performing arthroscopic repairs of the acetabular labrum. BMAC is a hopeful new complementary treatment in hip labral repair. However, due to the low quality and limited research available for this meta analysis, further high quality research is needed to better understand the biological mechanisms and long-term implications compared to standard treatment.
